FOrms

Before you are able to fully enroll your child in Mountaineers Programs, double check to make sure their youth forms are up to date. All youth participating in Mountaineers Programs need to have the following form(s) up to date:

• Youth Information Form

For youth participating in the following programs; Mini Mountaineers, Summer Camp, Pathfinders, Nomads, Explorers, Junior MAC, Mountaineers Adventures Club, and Family Programs they will also need:

• Youth Programs Form

For youth participating in adult programs they will also need:

•  Youth In Adult Programs Form
